The use of this application by Canada Credit Fix may be used for such reasons as initiating an account, membership or service plan. You are hereby authorized to obtain any information you may require for this purpose from other sources, including but not limited to credit bureaus (Equifax and or TransUnion) and credit collection agencies, and each source is hereby authorized to provide you with such information. I also understand, acknowledge and agree that the information given in the application form, as well as all other information you obtain in relation to my credit history may be disclosed to other service providers or institutions necessary for the evaluation of my credit history, including but not limited to technological and support services and any other parties with whom I propose to, or have been engaged in, a financial, credit or debt relationship. I further expressly acknowledge and agree that each service provider, or party I propose to have a financial relationship, or have been engaged in, to whom you provide the application or personal information from it, is permitted to receive such application and information and maintain records relating to me and my personal information and to hold, use, communicate and disclose personal information about me, including my Social Insurance Number (SIN), if I provide it, and collect/disclose personal information from me, you and from third persons, including credit bureau (Equifax and or TransUnion), credit reporting and collection agencies, institutions, financial institutions, my past/present employers, creditors, tenants, my spouse or any other person who has information about me for the purposes of recording, evaluating and responding to my application for credit profile evaluation or related activities and I specifically consent to the release and disclosure of personal information by such persons to and among you and each potential party or organization required to complete this process. I further expressly authorize and release from any liability, any mortgage broker, dealership of financier from whom I am currently communicating with, to release any and all personal information to Canada Credit Fix Inc., including but not limited, and credit bureau reports (Equifax and or TransUnion) that may have been obtained by them and providing such to Canada Credit Fix Inc. I consent to Canada Credit Fix sharing this collected information with affiliate agencies. I understand that by completing this application does not obligate Canada Credit Fix Inc or any of their affiliates to perform services or issue a membership either basic or premium. Canada Credit Fix reserves the right to refuse service and or membership.
